All New Patients (except massage, see below), or patient's returning after an extended period of 2 years, are required to fill out and complete a Patient Intake Form. These can be supplied at your visit, or to save time, you may print, complete, and bring it with you to your visit.
A price list is included with our cancellation policy. We require 24 hours cancellation notice for all services.
A consent form to receive treatment from a Registered Chiropractor is required and included in the intake forms.
