New Patient Forms

All New Patients (except massage, see below), or patient's returning after an extended period of 2 years, are required to fill out and complete a Patient Intake Form. These can be supplied at your visit, or to save time, you may print, complete, and bring it with you to your visit.


A price list is included with our cancellation policy. We require 24 hours cancellation notice for all services.


A consent form to receive treatment from a Registered Chiropractor is required and included in the intake forms.


For those interested in acupuncture, an acupuncture consent form is below.

Motor Vehicle Accident Patients


Please complete the above New Patient Intake Form with all required signatures. Also, make sure to visit our Motor Vehicle Accident page for information of what is required at your appointment. An OCF 3  is also required at the first visit.
