At Innova Wellness, patients are treated for many different injuries associated with car accidents, and guided through their physical rehabilitation process.
Before Coming to the Clinic
Make sure your case has been opened by reporting your accident to your auto insurance company. If you were a passenger in a car, make sure the driver has reported the accident.
Please call the clinic at (905)884-2121 for an appointment as soon as you can. We ask that if possible, please print and fill out a copy of our New Patient Intake Form.
We ask that you please bring a copy of your Disability Certificate (OCF 3 Form) with you to your appointment.
If you have yet to fill one out, it can be provided for you in the clinic. You will need your Policy number, Claim number, and Adjuster information.
If you have extended health insurance, please bring a copy of your card or certificate and group number.
The doctor will do a full assessment, needed paperwork, and discuss the best individualized treatment for you.
